While UCO attempts to update our resource list regularly, we are not able to monitor every change made by outside agencies. UCO Bylaws
– Welcome packets are available at the UCO office. Association Officers are encouraged to present new residents to their buildings with a welcome packet to so they can familiarize themselves with amenities, gate access and other helpful information about life in Century Village.


ASSOCIATION FORMS

This form serves to authorize voting at delegate assembly meetings and emergency contact details. It should be updated by the Association after their annual meeting and with any changes to delegates/officers. Associations should have at least one alternate delegate to ensure representation

Please note, to protect delegates’ voting rights, completed forms must still be printed, affixed with the Association’s seal, and turned in to the UCO Office in person by a board member. Emailed or incomplete submissions will not be processed.

Please use this brief form to update our records with the contact information for your Association’s management company.

POD Storage Authorization Form
Parking of POD storage units for more than one hour must be authorized in advance by the association board.

Unit Purchase, Rental and Occupancy Applications
Forms are being provided here for the convenience of CV residents. Completed applications must be submitted to the Condominium Association for board member signature(s). UCO will not accept applications that have not undergone this process. Realtors should direct all queries to the relevant Association Board.

SERVICE FEEDBACK/ASSISTANCE REQUESTS

Use this form to report unresolved Breezeline issues. This is an interactive form; once downloaded, users can choose to type or handwrite, per their preference. Completed forms can be left with the receptionist or an Officer at the UCO Office or submit it by email to cvtvadmin@unitedcivic.org.

Transportation Bus Feedback Form
Use this form to share feedback – complaints or compliments – about your experience using Century Village’s bus system. Completed forms can be left with the receptionist or an Officer at the UCO Office or submit it by email to transportation@unitedcivic.org.


VEHICLE & BARCODE RESOURCES

Barcode Desk
To obtain a barcode for your vehicle, bring you Century Village ID and current vehicle registration to the UCO office. Your CV ID and registration must be under the same name, without exception.

Barcode desk hours are Mon-Thu 9:00 am-11:30 am and Fri 12:00 noon to 2:30pm. The charge for a new barcode or replacement sticker is $10 (cash only); updates are free.

Florida Vehicle Registration
Registration renewals can be completed at the self-service DMV Kiosk located at Publix in the Village Commons: 831 Village Blvd, West Palm Beach, FL 33409. More info on Palm Beach County’s DMV kiosks: https://www.pbctax.gov/motor-vehicle/registration/kiosks/.
